I understand that all Events and Open plays must be located in the State of Wisconsin to be valid.

I understand this form will be used to receive my rebate from C.T.S. Sports. The transfer should take 3-5 business days after the customer has been contacted.

Must exceed $20 as well as can't exceed the customers membership program costs.
